Who it helps

Personalized check-ins, easy-to-understand guidance, and a team that knows you — between every visit.

We extend your care between visits with structured follow-up, education, and shared progress updates — without disrupting your workflow.

Stay in sync on shared patients with timely insights, adherence support, and condition-specific coordination.

We partner with senior centers, community organizations, and home-based providers to surround patients with support.
Conditions supported
REACH Care Now supports patients living with conditions across the three ACCESS Model tracks. Eligibility is determined on an individual basis.
Including diabetes, hypertension, heart disease, chronic kidney disease, and related conditions.
Including chronic joint pain, arthritis, mobility limitations, and post-injury recovery.
Including depression, anxiety, and the behavioral dimensions of chronic illness.
How it works
From the first conversation through ongoing support, every step is built to complement — not replace — the patient's existing care team.
A provider refers a patient, or the patient or caregiver reaches out directly.
We confirm Medicare eligibility and qualifying conditions under the ACCESS Model.
We listen — to health history, goals, daily life, and what matters most.
A tailored plan built with the patient and shared with their care team.
Coordinated check-ins, coaching, and resources between visits.
Transparent updates flow back to the care team so everyone stays aligned.

For providers & partners
Providers can refer eligible Medicare patients to REACH Care Now for coordinated support between visits. We're designed to complement, not replace, the patient's existing care team.
The bigger picture
REACH Care Now is the beginning. Over time, it will grow into the broader REACH Community Care Network — connecting primary care providers, specialists, community organizations, senior care partners, and virtual care teams to better support patients with chronic conditions across our region.
Care that is local, connected, and built around the people we serve.
